Documents required for filing an application:

Sports medical examination report AB1, not older than 4 weeks
AB1 completely filled in (no corrections!), stamped by a specialist, alternatively ward / unit with stamp, date and signature
internal part of an internist/specialist in internal medicine or a general practitioner, not older than 4 weeks
Orthopaedic part of orthopaedists or surgeons, not older than 4 weeks, sports physicians not recognised
All blood results must be submitted
Application for membership in the association ISPA e.V.
